Description

Primary Research Applications:

  • Pituitary Somatotroph Function & GH Secretion Studies
  • GHRH Receptor Signaling & cAMP Pathway Analysis
  • Comparative Research with DAC-Containing Analogs
  • Pulsatile GH Release & Metabolic Modeling

Tesamorelin, CJC-1295, and Ipamorelin are all growth-hormone (GH)–related peptides, but they differ in structure, receptor targets, duration, evidence, and typical use.

Tesamorelin

  • Synthetic full-length (44-amino-acid) analog of growth hormone-releasing hormone (GHRH).
  • Acts only on the pituitary GHRH receptor to stimulate natural, pulsatile GH release (and subsequent IGF-1 production).
  • Modified (N-terminal protection against DPP-IV) for a plasma half-life of ~26–38 minutes; requires daily subcutaneous injection.
  • Only FDA-approved peptide in this group (as Egrifta, 2010) specifically for reducing excess visceral abdominal fat in adults with HIV-associated lipodystrophy. Strong Phase 3 RCT evidence shows ~15% visceral fat reduction (CT-measured) over 26 weeks, plus improvements in liver fat and triglycerides in that population. Often discussed off-label for visceral fat/metabolic goals.

CJC-1295

  • Synthetic GHRH analog based on a truncated (modified GRF 1-29) sequence.
  • Also acts on the GHRH receptor to amplify GH pulse amplitude.
  • Two main versions:
    • No-DAC (or Mod GRF 1-29): half-life ~30 minutes; pulsatile effect; typically injected daily (often 1–2×).
    • With DAC (Drug Affinity Complex): binds albumin; half-life ~6–8 days; more sustained/tonic effect; less frequent dosing.
  • Not FDA-approved (reached early clinical trials only). Used in research/compounded settings, commonly stacked with a GHRP for broader GH support, recovery, sleep, and body composition. Limited human outcome data compared with tesamorelin.

Ipamorelin

  • Selective growth hormone–releasing peptide (GHRP)/ghrelin mimetic (pentapeptide).
  • Acts on a different receptor—the ghrelin receptor (GHS-R1a)—to trigger discrete GH pulses.
  • Highly selective: stimulates GH with minimal or no elevation of cortisol, prolactin, or strong appetite effects (unlike older GHRPs such as GHRP-2/6).
  • Half-life ~2 hours; typically injected 1–3× daily.
  • Not FDA-approved. Frequently paired with a GHRH analog (especially CJC-1295 no-DAC) because the dual pathways (GHRH + ghrelin) produce synergistic, more robust GH pulses while remaining relatively “clean.”

Key contrasts

Aspect Tesamorelin CJC-1295 Ipamorelin
Receptor GHRH only GHRH only Ghrelin (GHS-R1a)
Half-life ~26–38 min ~30 min (no-DAC) / 6–8 days (DAC) ~2 hours
FDA status Approved (specific indication) Research/compounded Research/compounded
Strongest evidence Visceral fat (HIV lipodystrophy RCTs) Early PK/PD studies Selectivity & GH-release studies
Common pairing Sometimes with Ipamorelin Very often with Ipamorelin With CJC-1295 or other GHRH analogs

In short: Tesamorelin is the evidence-backed, single-pathway GHRH analog with a clear medical indication for visceral fat; CJC-1295 is a (longer- or shorter-acting) GHRH analog mainly used in stacks; Ipamorelin is the selective “pulse amplifier” that complements GHRH analogs via a second pathway. All raise endogenous GH rather than supplying exogenous GH, but only tesamorelin has robust Phase 3 human data for a specific clinical outcome.
