Description

Methylene Blue (MB)

  • A synthetic phenothiazine dye, first synthesized in 1876; historically notable as one of the first fully synthetic drugs used in medicine.
  • FDA-approved uses: treatment of methemoglobinemia (it acts as an electron acceptor/donor, reducing methemoglobin back to functional hemoglobin), and it’s used off-label/adjunctively for vasoplegic syndrome and as a surgical/diagnostic dye (e.g., mapping lymph nodes, parathyroid tissue).
  • Mechanism: at low doses, it acts as an alternative electron carrier in the mitochondrial electron transport chain, bypassing Complex I/III and reducing oxidative stress/reactive oxygen species in some contexts. It’s also a monoamine oxidase inhibitor (MAOI) at typical clinical doses — a clinically important property since it can cause serious serotonin toxicity when combined with SSRIs, SNRIs, or other serotonergic drugs.
  • Investigated (not established) uses: cognitive enhancement/nootropic use, and Alzheimer’s disease via a derivative called LMTM (formerly known as TRx0237/Rember), studied for inhibiting tau protein aggregation. LMTM went through Phase 3 trials; results were mixed and it did not clearly demonstrate benefit over the active comparator in the primary endpoint analysis, so it is not an approved Alzheimer’s treatment.
  • Evidence quality caveat: Much of the “nootropic/longevity” enthusiasm around methylene blue circulating currently draws on preclinical (cell/animal) data, small mechanistic human studies, or promotional sources rather than large, rigorous human RCTs — some online claims (e.g., about dramatic slowing of Alzheimer’s progression) overstate what the actual trial data showed. Regulatory status for these off-label cognitive/longevity uses is unapproved in the US; compounded/supplement-grade methylene blue is not FDA-regulated the same way as pharmaceutical-grade product.
  • Known risk to flag as fact, not advice: its MAOI activity creates a well-documented serotonin syndrome interaction risk, which is pharmacologically distinct from most other peptides/compounds.
